Qualifications

- Valid ID, background clearance, and TB clearance

Deliver skilled nursing care in home and facility settings, and help shape individualized care plans.
Responsibilities
- Complete assessments and develop care plans
- Administer medications and treatments as ordered
- Supervise LPN/LVN and home health aide staff
- Maintain accurate, timely clinical documentation
Qualifications
- Active, unrestricted RN license in California
- Current BLS/CPR certification
- Home health or acute care experience preferred
- Strong clinical judgement and communication
Licensed Practical / Vocational NurseFull-time / Part-time · Burbank, CA & surrounding areas⌄
Provide hands-on nursing care under RN supervision, supporting recovery and chronic condition management.
Responsibilities
- Monitor vital signs and document patient status
- Administer prescribed medications and wound care
- Support clients and families with care education
- Coordinate with the RN and care team on plan changes
Qualifications
- Active LPN/LVN license in California
- Current BLS/CPR certification
- Experience in home care or long-term care preferred
- Detail-oriented with strong documentation habits
Certified Home Health Aide (CHHA)Full-time / Part-time · Burbank, CA & surrounding areas⌄
Support clients with personal care and daily living activities while promoting dignity and independence.
Responsibilities
- Assist with activities of daily living
- Support safe mobility, transfers, and exercises
- Maintain a clean, safe home environment
- Document care provided each visit
Qualifications
- Current California CHHA certification
- Current BLS/CPR certification
- Background clearance and TB clearance
